Our intake procedures have changed.

The first step for possible admission to our recovery home program is to fill out an electronic application either here on our website or in our office.  Applications can be filled out in our office on Tuesday afternoons at 1:30 pm with an orientation/assessment following at 2:15 pm. If you fill out the application and submit online it is still necessary to attend the 2:15 pm orientation/assessment to complete the application process. No appointment is necessary for either the 1:30 or the 2:15 portion of the Tuesday application/orientation/assessment process.  Phone 630-966-9000 if you have any questions.

Clients coming from a treatment center should bring with them a biological; psychological; social history, and current TB test results. Regardless of where a client is coming from a current TB test is required before admittance to our residential program.
